An authorization letter is a written confirmation that gives temporary or long-term authority to another person to act on behalf of another person. Though you can find lots of useful templates on Google, the most important thing is to know that it is a formal document, so it should be polite and professional. You always have to put your most important personal data on it, such as your name, address and the actual date. In the form of address, it is best to use the recipients' name, or if you don't know that, the "To Whom It May Concern" form.
